Health care system in Colombia is public and universal. Unless you're a beneficiary of the subsidized system (Sisbén), contributions are paid by the employer (8% of salary) and the employee (4% of salary). These contributions are mandatory. The money is then redistributed by the government. Higher income population still pay private insurance ("prepagada") to access special treatments, skip the lines, and hardly ever use the universal public health system.
