I've seen that index is higher in really polluted cities. But not much higher than the real difference. Maybe, some compromise would be moderate, I'm concerned and moderate, I'm not concerned (because of moderate emission factories providing employment for example), where moderate not concerned option is 40 points and moderate concerned is 50 points of pollution index.

This rating is a bit skewed. There are concerned and not concerned options, while low to high would be more objective. For example, there are catalizator issues with cars, it means that peiple are concerned and what changes from government to better control car market. But this is moderate pollution, and this index is the same as in highly polluted cities. High pollution is some type of factories, plants, as in some Chinese
and East asian cities,and Krasnoyarsk Russia, or pollution of extremely high density poor living leading to extreme emissions like India, or pollution from excessive fossil fuels usage, etc.
